Linda Herbert

Corporate Director of Care Management Post Acute Services

Linda Herbert is Corporate Director of Care Management for Post-Acute Services. In this role she serves as a resource to CHC’s long term acute care hospitals to ensure appropriate and quality care, provide case management strategies, and cultivate ongoing quality assessment and performance improvement activities. She also participates in the selection and professional development of case management staff.

Prior to joining CHC, Linda spent more than five years with Kindred Healthcare where she served as Area Director of Case Management. There she was able to turn around four hospitals in the Denver market within 12 months.

Linda holds a Master of Arts in Bioethics and a Master of Science Social Administration from Case Western Reserve University in Cleveland, Ohio. She is also a licensed social worker in three states and has an advanced certificate in hospice and palliative care.
